Notarized Parent/Guardian Affidavit Information 

· When a student is permanently residing apart from his/her parent(s) or legal guardian(s) in a dwelling that is owned or leased and occupied by a legal Rocky Hill Resident (called the Host hereafter), a Notarized Parent/Guardian Affidavit is required to be completed by the student’s parent(s) or legal guardian(s). · Board of Education Procedure requires the host to present proof of residency in Rocky Hill during the school registration process.  For further information on residency, please visit the Residency Requirements site online here. 

· All students attending Rocky Hill Public Schools under a Parent/Guardian Affidavit are subject to residency investigations which may include unannounced residency visits and surveillance at any time.  

· All Parent/Guardian Affidavit’s expire on the last day of school 2013.  For continued enrollment in the 2013-14 school year, the parent(s), guardian(s) must file a Parent/Guardian Affidavit Renewal if the student continues to reside with the host. Renewals will be mailed in May 2013.  Failure to renew will cause student to be inactivated from RHPS. 

· All Parent/Guardian Affidavits are reviewed by the School Residency Coordinator.  For questions please call (860) 258-7701 Ext. 177.
